I e-mailed the original poster and asked him about the wife's name, and he got right back to me. I then spent over an hour looking for this family and uploading the document, and I got this message back from the op:

"Thank you, Mary. Several people have already sent this."

I'm not sure why he didn't say something the first time I contacted him, but I wanted to make sure no one else wastes time on this request.
