Thank you for the information. I didn't have this census report. This could be my Anna looks like. I found other KY records of an Anna M that I thought could be my Anna (David's mother), but wasn't sure. A David C lived at the same address with her. So this could be her. Gives me more to work with. I'll follow up with this.
As for the death certificate. I had been told from other sources that David's father was William and his mother may have been Sarah or Sarah Ann or something along those lines. Maybe that source originally was trying to say his mother was Ann and her mother was Sarah since it appears so in some census reports I found. I just hadn't found this one you have listed here. Thanks again for the information. I will follow up on it. Where did you find this census report? I haven't found it on Ancestry or My Heritage or the other sites I have used. Do I just need to try a general Google search for Nelson Co, KY records?
