My Kinsey heritage is in Jefferson Co., Fl. My understanding is that they originated in S.C. The Kinsey graveyard here has some of the original settlers from the early 1800's of which I believe one was William.......Not 100% sure but will check next time there.
My grandfather is Clarence Cherry Wells. His father was Ira J Wells. Ira's mother was Lucinda Kirkland. Lucinda's parents were Henry Kirkland and Celia Barrentine. My mother remembers going to Ira's house and in a shed in teh backyard he had rattles and powders. He was a Seminole medicine man. I have also ways been told that Ira's mother was full-blooded Seminole Indian. Not sure though.

I am so sorry to wait so long to reply but I moved and have been busy. I have tried several times to go up that road but sometimes a police car is parked there. I don't know what was going on. Then the road itself had a "do not enter" sign. So I guess the cemetery is still there.
Can you give me any more information and perhaps one day I will be able to visit their grave again.
Thanks! I look forward to hearing from you.
mwilson1693@cfl.rr.com

Suggestion: get a copy of your father's Social Security application (details below). Your mother's too, if needed. Also perhaps the marriage record.

Good luck!

To start:

Social Security Death Index
Name: Leroy Carroll
SSN: 244-74-2799
Born: 5 Nov 1947
Died: Feb 1984
State (Year) SSN issued: North Carolina (1963)

Source Citation: Number: 244-74-2799; Issue State: North Carolina; Issue Date: 1963.

Source Information:
Ancestry.com. Social Security Death Index [database on-line]. Provo, UT, USA: Ancestry.com Operations Inc, 2011.
Original data: Social Security Administration. Social Security Death Index, Master File. Social Security Administration.

Description:
The Social Security Administration Death Master File contains information on millions of deceased individuals with United States social security numbers whose deaths were reported to the Social Security Administration. Birth years for the individuals listed range from 1875 to last year. Information in these records includes name, birth date, death date, and last known residence.

© 2012, The Generations Network, Inc.
<< Return to Results Screen

Writing a letter to the Social Security Administration

When you find your ancestors in the Social Security Death Index (also known as the Death Master File), you may want a copy of your ancestor's original Social Security application. This application has valuable additional information and can serve as documentation for your own records. The Social Security Administration makes copies of the original Social Security application form (the SS-5) available to third parties who request information on a deceased individual.

The application form (SS-5) contains the following information:

Full name
Full name at birth (including maiden name)
Present mailing address
Age at last birthday
Date of birth
Place of birth (City, county, state)
Father's full name "regardless of whether living or dead"
Mother's full name, including maiden name, "regardless of whether living or dead"
Sex and race
Ever applied for SS number/Railroad Retirement before? Yes/No
Current employer's name and address
Date signed
Applicant's signature

A standard letter to the Social Security Administration is available with the search results on the Ancestry.com Social Security Death Index. This letter may be printed and mailed to the Social Security Administration to request a copy of your ancestor's SS-5 form. The Social Security Administration now charges $27.00 for each individual copy (an abbreviated NUMIDENT printout is available for $16.00).

Social Security Administration
Office of Earnings Operations
FOIA Workgroup
300 N. Greene Street
P.O. Box 33022
Baltimore, Maryland 21290

Click here for help generating a letter
Note:

Not everyone who has, or has had, a Social Security card will appear on the Social Security Death Index (SSDI). The SSDI lists only those individuals for whom a lump sum death benefit was paid. Many other enrolled persons are not listed either because their death was not reported or they are still living. If you haven't found your deceased ancestor on the SSDI, but he or she was likely enrolled with the Social Security system, it may still be possible to get a photocopy of his or her original SS-5.

To request this information, you will need to send the name and Social Security number of the person you're researching. You might be able to locate the person's Social Security number in the following places:

personal papers
death certificate
funeral home records
records held by financial institutions
voter registration rolls at county courthouse
former employers of the individual

If those sources don't produce the number, you may request a "records search" with the Social Security Administration. To request this service you must send $29 and provide the following information: full name, state of birth, and date of birth to:

Freedom of Information Officer
4-H-8 Annex Building
6401 Security Boulevard
Baltimore, Maryland 21235

Providing names of parents are also helpful, especially with common surnames. Be sure also to provide proof of death, as the records of living individuals are not publicly available.

Contact Us
Ancestry.com Blog
Affiliates
Gift Memberships
Careers

Visit our other sites:

© 1997-2012 Ancestry.com | Corporate Information | New Privacy | New Terms and Conditions

Social Security Administration
Office of Earnings Operations
FOIA Workgroup
300 N. Section 552. I hereby request a copy of the SS-5, Application for Social Security Card, or a corresponding NUMIDENT printout (see below) for the following individual:

Carroll, Leroy
244-74-2799
Birth: 5 Nov 1947
Death: Feb 1984

This individual is deceased, having been listed in the Social Security Administration's Death Master File. I understand the fee for this service is $27 for copy of original SS-5 application OR $16 for copy of an abbreviated NUMIDENT printout when the Social Security Number is provided. I am requesting a (circle one: SS-5 copy OR Numident printout) Included is a check for $______ made out to the Social Security Administration to cover any administrative costs required by this request.

Please respond to my request upon receipt of this initial correspondence. Thank you for your attention and assistance.

Sincerely,

Address:

Daytime Phone Number:

Florida Marriage Collection, 1822-1875 and 1927-2001
Name: Leroy Monroe Carroll Jr
Marriage Date: 23 Jul 1977
County of Marriage: Palm Beach

Gender: Male
Race: White

Volume: 4117
Certificate:
Source: Florida Department of Health

Source Information:
Ancestry.com.
Help me find my birth family!

I was born in Palm Beach, FL to Leroy M Carroll and Deborah L Carroll, have a blood brother Leroy D Carroll. My birth name is Christina Lynn Carroll, later adopted to "Jones". My father died 3 Feb 1984 and mother died 21 Jan 1984. I was tossed around and later adopted by my maternal grandparents (Hubert and Dolores Jones). My adoptive family never spoke of my father's family and I'm not sure why. I've never met any of his family. My parents are both buried in Palm Beach, Florida. I have my father getting his SS Card from North Carolina, so I'm assuming that his family would be from NC or FL. I know that my birth father had a son from a previous relationship, last name of Carroll, not sure if he is living but he was at least 5 years older than I. I don't speak to my adoptive family for numerous reasons. I would really like to get in touch with anyone who may know where I can find my blood brother and father's living family. I'm in the Air Force and currently serving in Louisiana. I recently found out about ancestry.com and hope I can reach out to someone. My email is clynnd821@gmail.com, any help or direction with this would be greatly appreciated. Thank you!

Popena, Perpena, Papina and various other spellings

What I found, so far, is that blacks with the surname of Perpena, and its various spellings, are endemic to FL until after 1870. Between 1840 and 1860 blacks by this surname were living in Key West. In 1830 there 2 women by this surname in St. Johns county. In 1870 there was an "explosion" or people with this surname in Key West, St Augustine, and Fernandina. In addition to 1 man who was born in FL but moved to VA. I'm related to the Fernandina Perpena's. Is there way to link and/or eliminate the others? I've been looking at Civil War documents and census information, but I can't find any information that links my great great great grandfather to the Key West or St. Augustine Perpena's. What should I do next?

Sorry, I don't find these names in my tree. By the 1930 time frame, the Morgan's and Gillette's that I have listed, are all living in Florida.

Wish I could be of more help to you. I think it is interesting that the last names are similar to those in my family (as well as, the native American connection). But, at this point, I think that it is only coincidental.

Have you tried to get your grandmother's death certificate? It should list her parents names.

Good luck with your research

Hello Erika,

My name is Conrad Ray Laughinghouse, I am a retired Air Traffic Controller living in Red Rock, Texas just outside Austin. I saw your post GenForm from 2001 and wanted to do some follow-up. My wife is Linda who started at least one of the treads on GenForm related to Laughinghouse.

If you remember the GenForm posting was about a Lulu (Lula) Laughinghouse of Jacksonville, FL. Lulu was my aunt and raised my dad. Dad moved to Cleveland when he was 21 or 22 and I was born and raised there.
In the early 70s I was stationed in Little Rock with the FAA and met someone who told me there was a Laughinghouse family living in Forest City. I was an amateur radio operator and once, while passing through, I talked to a ham in Forest City and gave him my last name and asked about a Laughinghouse in the city. He laughed and said his bank officer was named Laughinghouse and the officer's son delivered his newspaper.

Unfortunately I never followed up on the information, would this be part of your family?

I just started a group, "Laughinghouse," on Facebook, sadly I lowered the average age on Facebook by about three years when I joined. ;> ) Haven't done much with the group as yet.

I would love to here what you have been able to find out about the Laughinghouses.

Sincerely….Ray Laughinghouse linray2@gvtc.com (Facebook: Conrad Ray Laughinghouse)
Suggestions:

1. Study (very carefully) the original image of the 1900 Federal Census for Florida - Hamilton County. Looks like John was 26, born April 1874 in FL; Lucy was 17, born 10/82 in FL; James was also 17, born 10/82 in FL.

Consider this possibility: James was not a WYNN - instead, he and Lucy were twins. Lucy probably gave the info to the census enumerator - she likely said that James was a "brother", without specifying that he was HER brother, not John's, and the enumerator assumed that James' surname was WYNN.

2. Use a Soundex spelling when searching: WYNN and WINN are interchangeable.

3. Look (again, very carefully) at the earlier 1880 Federal Census for Florida - Hamilton County. You'll find John with two brothers living with their mother and grandmother. Check the marital status and birth places of the women.

4. The mother and the two brothers also show up in the 1900 Hamilton County census. Didn't check the grandmother.

5. Consider this name: James Byron PROCTOR, born 10/23/1882. He appears (indexed as "PROCTON") in the World War I Draft Registration for De Soto County, FL. In 1920 he's nearby in Brewster, Polk County, FL with his children and his widowed father.

This MAY (or may not) be Lucy's brother and father.

Good luck!

Hello

I am looking for info on John David Wynn and his wife Lucie (or Lucy) Minerva. They were married in 1899, possibly in Hamilton County.

The 1900 census shows them in Octahatchee. John D was then 21, Lucy was 17 and their first born was seven months old. John David's brother James, also 17, lived with them at this point.

They are my grandparents.

By 1910 they had relocated to DeSoto County and had had three more children, including my father, Hugh Vernon Wynn (born 1907). Apparently the first child did not survive.

Specifically I am interested in Lucy's maiden name; and the name of my great grandfather, John David's father. From what I can tell Lucy was born October 1882.

Any suggestions regarding research resources for Lucy Minerva's background also very much appreciated.

Many thanks in advance
Sylvia
coach_sylvia@yahoo.com

Question: Let's us say that it is the 1890's. What would have been the method or best way from getting from New Orleans to Palmetto ? Railroad or by boat ? My great great grandfather Thomas Minnock stole money from his employers, H and C. Newmann in New Orleans and fled to Palmetto. I always wondered how he would have traveled.

Kathleen Harrison was my Aunt, which would make me your cousin. My father was her brother Bill. I do not know much about her except she was killed in an automobile accident in Florida in 1957 and had two sons. I only recently discovered the names of her two sons, who are my first cousins. One of them is your father, I assume. I know it has been many years but I am trying to locate them and their family members as we have never met.
