Helping someone with their family tree and have a copy of birth certificate from Jefferson County, dated 1926.
Very hard to read some areas and need to know what the form says in some places (I can read the handwriting just not the typeset form).
At the top it says Jefferson and just under State Board of Health is Pag. ????? with a stamped number 6-digits. It looks as if there is another line directly under that also something to the right of it.
Under that is Birmingham Ala. at (address) ?????______
Name of child______________________
Darker horizontal line and under it
??????? B (for boy?)????? ??????? Are Parents Married-Yes followed by Date of birth.

Can anyone tell me what those areas say on the form? In some places it is blank and others very faded or unreadable.
