Registration of Death / Long Form Death Certificate
From Provincial Govt. Vital Statistics Dept. There is a Fee.
Order the
REGISTRATION or Death / Long form Certificate -
it has all of the personal information as required at the time. as known to the informant.

Place of Death - Name and Address - Date and Place of Birth - Sex,, Race, Citizenship
Marital Status - Name of Spouse
Names of Parents including Maiden name of the mother - Place of birth of parents.
Date of Death - Burial or Cremation -
Name and Address of Funeral Home in Charge of Arrangements
Name and Address of Cemetery
Note: Cause of Death - might not be included in the registration - it may require a request for
Physicians Report Cause of Death - or something similar.

The Short Form Certificate: - Is an EXTRACT - of Basic Information taken from the Registration Good for Proof of the Event - without disclosing personal Information to those with no need to know.
