Deciphering address on ship notes

Surnames: Hellman
I'm trying to figure out if I can get a more specific address for some of my ancestors that left Poland in 1922. Their names were Sarah Hellman with her 2 kids Carl & Edith Hellman.
Under "The name & complete address of nearest relative or friend in country where alien lived" It has:
Uncle Mr. J Karpp, Strodkowe
36. Stanislawow, Poland

Could Strodkowe be the name of the road? What does 36 mean? House #?
And why did they have to list this?

Also right above the language "Polish" they can read & write - there is a handwritten number which I believe reads 3/0643 - which seems to be written above a lot of other people's names. Any idea what this means?

Last question - any suggestions the best place to locate an exact address for them in Stanislawow?
