The family is indeed from Smolnik - a village in the administrative district of Gmina Komańcza, within Sanok County, Subcarpathian Voivodeship, in south-eastern Poland, close to the border with Slovakia. It lies approximately 10 kilometres (6 mi) south-east of Komańcza, 32 km (20 mi) south of Sanok, and 85 km (53 mi) south of the regional capital Rzeszów.

My ancestor's father is Simeon (Sam) Kosiak:
Simeon Kosiak was born about 1835 in Smolnik. With his first wife Anne Diidik (b. 1839), he had following children (all born in Smolnik): Katarina (Stillborn), Frank, Anna, Tatianna, Eudokia (Eva), Elias, Maria, Euphrosina, and Julianna.
After his first wife died, Simeon Kosiak remarried and had the following children (all born in Smolnik) with his second wife Pelagia Ochnycz (she was from Lupków): Maria, Stephanus, Herasimus (Max) Kosiak, Maria, Joannes (died as infant), Michael, Nicolaus, Joannes, Stephen, Iwan (John), and Petro (Peter).
