The format is roughly "on Date, to/for Parent, Child" (with the child's name with the -t suffix marking it as the object of the sentence, which is a bit of grammar that English doesn't bother with). The parent's name on this one line is frustratingly hard to read, but it looks like just one name, rather than the surname + given name of the rest of the records on the page.

Without knowing which of the more than half-dozen places named Tótfalu ('Slovakville') this is, I can't go looking at the previous and next pages to see what format was used for illegitimate births, but that's my first thought for this entry. The parent's name looks vaguely like Tresanka, which could be a diminutive of Teréz(ia) = Teresa. (The last three characters are almost certainly _-nak_ 'to, for'.)
